I, the undersigned, hereby serve notice that I intend to vacate the above mentioned premises on the date stated here. I understand that according to my lease agreement unless otherwise agreed to in writing, if I am vacating prior to the last day of the month, I am still responsible for the rent through the end of the month.

Reason for Vacating

(Note to Tenant: If you have fulfilled all the terms of your lease you may anticipate a deposit refund. However, in order to receive a complete refund, you must comply with the following items, as well as the items listed on your lease agreement. Keep in mind that your liability is not limited to the amount of your security deposit.)

I understand the following:*

1.Any changes to the listed move out date must be agreed upon and confirmed in writing
2.Security deposit funds may not be used toward any rent payment due prior to the end of my lease term
3.All keys (including mailbox, storage shed, etc) and garage remotes must be returned by the last day of the lease term
4. The leaving of keys in the APS lockbox provided, handing them over to the property manager, or leaving them in the APS office drop box is considered vacating the property
5.During the last 60 days of your lease in-person showings to prospective residents Please choose at least two days during the week with either a morning or afternoon preference that are most convenient for you for showings to be conducted. You will be notified in advance of any scheduled showings

Morning show times are 9 - 1 and afternoon 1 - 6 Showings will be scheduled in advance and will be accompanied by an APS team member. You do not need to be present.
